This information is for parents/guardians of student athletes that attend Eagle View, Mountain View, 9th grade Academy, and the High School campus

All physical paperwork, other than section 6*, must be completed online through the Rank One System. No paper copies will be accepted. * Section 6 will be uploaded once it's completed by the physician

The CIPPE may NOT be authorized earlier than May 1st and shall be effective, regardless of when performed during a school year, until the latter of the next April 30th or the conclusion of the spring sports season. (Please refer to section 1 of the CIPPE forms on Rank One for further clarification).

Athletic Physical Forms for the 2025-2026 school year

Families will need their student's ID#, which can be found in their Skyward account

Forms to print and bring with you:

Once you have completed the section 5: Health History* on Rank One, please print this form and bring it with you to in-house physicals or your doctor's office. If you don't, you will have to refill this information out before your child is seen. *The Health History form is usually not needed if you are bringing your child to their PCP because they already have their health history. If you are intending to bring your child to an urgent care facility, or in-house physicals they will not complete a physical without this form completed.

Please bring this with you to in-house physicals or your doctor's office to be completed. The link to these forms is here: Section 5: Health History Page and the section 6: Medical Examiner form are the only forms that can be accepted for a PIAA sport, well child visits cannot be accepted.

Important Due dates for Rank One paperwork to be completed for the 2025-2026 school year:
Football- August 1st
All other Fall sports- August 8th
Winter Sports- Nov. 17th
Spring sports - Feb. 27th
