To enter Kindergarten children are required to have 4 DPT's, 3 Polio, 3 Hepatitis B, 2 MMR's and 2 Varicella (chicken pox). There are some exceptions.

To enter Seventh Grade children are required to have the previous mentioned immunizations plus, a Tdap and a Meningococcal vaccine.

 * The 4th DPT should be given after the 4th birthday. If it is given prior to the 4th birthday, your child will receive a 5th DPT.

** An Influenza injection should be given to all children yearly

*** Physicians are now giving Hepatitis A to children. At this time this immunization is not required to enter school.

*Tdap--minimum age 10 years for Boostrix and 11 years for Adacel.

**HPV (Human Papuillomavirus Vaccine)--minimum age 9 years. Administer 1st dose to female patients at age 11-12 years. Administer 2nd dose 2 months after 1st dose and the 3rd dose 6 months after the 1st dose.

***MCV4 (Meningococcal Vaccine)--administer at age 11-12 years and at age 13-18 years if not previously vaccinated.

****Varicella Vaccine--administer 2 doses to persons aged 13 years or older at least 4 weeks apart.
