Online form reminder and clarification

Dear CV Parent/Guardian,

We are writing with a friendly reminder, if you haven’t already done so, to please log into Skyward’s Family Access Center (https://skyward.cvschools.org) and complete the online forms for each of your students before the school year begins. Completion of these forms prior to the first day of school will help us to ensure the health, safety, and welfare of your student(s).

Upon successful completion of each form, you will see a new tile appear on your Family Access home screen with the header of either “submitted” or “completed.”

Submitted: This new tile indicates to you that the form has been submitted and indicates to our staff that the information is ready for internal review. Once reviewed/approved by our staff, the secondary tile will be removed from your view and your updated information will be reflected in the Family Access Center.

Completed: This means the information is automatically approved and has been reflected in the Family Access Center. This tile will remain in your view.

Please note that you only have to complete each form once for each student. However, medical and emergency forms will be available all year and will show “new” in the green bar so that you are able to update this information as necessary at any time throughout the year.

You are able to view the history and processing of all of your forms from the main menu by selecting “online form” and “my online forms history.”
