Middlesex Game Show- Parent Survey

Greetings Middlesex Families and Friends!

For our celebration on the last day of school, we will be hosting a game show. It will be teachers vs. students! This game show will be "Family Feud" style, posing specific questions, and listing the top choices in each category. This is where you come in!

We are asking families, students and staff to contribute to our survey. The more respondents we have, the better the game show will be. Please click on the link below, and fill out the survey. Remember to indicate whether you are a student (choose a grade), Staff Member, or Other (families, friends, etc.). Students will also be bringing home a BLUE paper for those who do not have access to a computer and/or internet. Students can place these papers in the school bus lobby (look for the big box).
