Locker request form for seniors

Attention parents/guardians of 12th-grade students:
 
Beginning this school year, seniors at CV High School will be required to opt into a locker. If you are a senior and wish to have a school locker for the 2018-19 school year, please click here to complete a locker request form. Lockers will be assigned to seniors as soon as possible and, once assigned, will be visible through both the CVSIS parent and student portal under the schedule tab.
 
Please note: Students in grades 9, 10, and 11 have been assigned lockers as per the normal process.
